Participation Release & Conduct Agreement

I give my child permission to participate in Liberty Township Baseball Association activities. I assume all risks and hazards incidental to participation, including transportation to and from activities.

I waive, release, absolve, indemnify, and agree to hold harmless Liberty Township Baseball Association, its organizers, sponsors, supervisors, participants, volunteers, and persons transporting any child to and from activities for claims arising from injury or death, except to the extent covered by accident or liability insurance.

I agree to abide by LTBA rules, regulations, and the Parent/Fan and Player Code of Conduct. I understand that once a player has been placed on a team, refunds are not issued unless a team cannot be formed for that age bracket because of insufficient players.
